How to get started with hospice

Starting hospice is simpler than many families expect. One phone call starts the process, and we guide you through every step.

How to get started with hospice care

Four steps to start care

  1. 1

    Call us

Tell us about your loved one. No referral needed to ask questions.

  2. 2

    We confirm eligibility

We work with their doctor and check coverage.

  3. 3

    We visit your home

A hospice nurse meets your family and builds the care plan.

  4. 4

    Care begins

Visits, equipment, and supplies start, with a nurse on call 24/7.

What to have ready

These help us move quickly, but you don't need everything to call.

  • Insurance cards (Medicare, Medicaid, or private insurance)
  • Your loved one's doctor's name and phone number
  • A list of current medications
  • Advance directive or power of attorney documents, if you have them
  • Any questions you'd like to ask

Don't have everything? Call anyway.

We can gather records and talk with your loved one's doctor for you.

What happens at the first visit

A hospice nurse comes to your loved one's home to get to know your family and set up care. Bring your questions. There are no wrong ones.

  • Meet your hospice nurse
  • Review medications and symptoms
  • Talk about your loved one's wishes and goals
  • Sign the consent forms to start hospice
  • Plan visits, equipment, and supplies
  • Get the 24/7 number to call any time

Who can start the process?

Anyone can call us: your loved one, a family member, a friend, or a doctor. We work with your loved one's physician to confirm eligibility, so you don't have to arrange it yourself.
